A care plan is a structured plan your GP prepares with you for a condition expected to last six months or more. It sets out your health goals, the treatment and services you need, and who is involved in your care, and it is reviewed regularly so your treatment keeps pace with your condition.

A care plan can also give you access to Medicare-subsidised visits to allied health providers each calendar year. Because our physiotherapy, podiatry, chiropractic and diabetes education services are on site, your GP and your allied health team can work together under one roof.
